SERVICE - Health inequalities & prevention

Strategies that shift the wider determinants - and the outcomes that follow

Health inequalities strategies fail more often than they succeed - not because the science is wrong, but because the strategies never bridge the gap between data and delivery. We help systems build inequality and prevention strategies that clinicians, commissioners and communities can actually put into practice.

What we do

What we do

  • Health inequalities diagnostics - evidence-based diagnostics of the inequalities in your population, their drivers, and the levers you actually have.
  • Core20PLUS5 and equivalent frameworks - grounded, practical application of national frameworks to local context, without box-ticking.
  • Prevention strategy - clinical and non-clinical prevention strategies that link population data to delivery capacity, workforce and finance.
  • Wider determinants strategy - working across health, local authority and VCSE partners on the determinants of health that sit beyond the NHS.
  • Inequalities-informed service design - building equity considerations into service redesign, pathway change and access strategies from day one.
  • Impact measurement - outcome frameworks that let boards see whether an inequality intervention is actually working.
How we work

How we work

Inequality work fails when it lives in a separate strategy document nobody references. Ours is designed to live inside your service strategy, financial plan and delivery programmes - not alongside them.

We are realistic about what the NHS can and cannot change. Wider determinants matter enormously; NHS influence over them is real but bounded. We help clients focus effort where it will move outcomes, and partner properly where it will not.

We work closely with local authority, public health and VCSE partners. Some of our best inequality engagements are led jointly with a Director of Public Health or a place-based partnership.

What to expect

What you can expect from us

  • A senior lead with real experience of NHS inequalities work - not a generic strategy consultant.
  • Recommendations that fit inside your delivery cadence, not a separate parallel programme.
  • Honesty about which interventions the evidence supports and which are, at best, unproven.
  • A measurable link between the strategy and the change you are trying to make.
Engagements

Typical engagements

  • An inequalities diagnostic and strategy for a place-based partnership.
  • A prevention strategy for an ICS, linked to workforce plans and financial envelope.
  • A Core20PLUS5 delivery plan for an NHS trust, embedded into operational planning.
  • Inequality-informed redesign of a specific service pathway.
